You have to pinch the end when putting it on so there's no air in it. That can cause it to tear, also if you have wrinkles in it or when you punch a hole through it. All have been done before, believe it or not

Some guys are stupid enough to try and beat the odds, so they put on more than 1 condom at once, hoping that the probability would be additive. However, what it actually does is introduce friction between the two condoms, increasing the likelihood of tearing them instead.
