SINGLE PLAYER.

So I know its dumb, but I wanted to know about Arthur getting TB, more precisely about what happens when not following the main missions? I want to explore the world, but don't want him coughing and dying while doing so, It would be agonizing, I want to know if it will happen while I play the sidequests to avoid it and explore before he gets sick. Basically I wanted to know if there is a problem in free roaming with the character being sick by the main story progression

The coughing is only in the custecenes?
