Plottwist for the Isekai is that actually is a different world although we assume the main character is off their rockers initially and just was hit by a car.

Holy shit, imagined if it seem like the exact same world at first but slowly reveal itself to be different. It is some r/retconned bullshit

It starts off the same, only the MC has an unexplained feeling that they're right. Over the course of the series, we see more and more historical & cultural discrepancies untill suddenly it goes full sci-fi or magic.

Or, a comedy series where all the absurd logic & creatures are being revealed at full force and are treated as normal by everyone else but the isekai'd MC.
