What is probably happening is that you are applying too much pressure on your cock. When you are cumming, you need to release some of the pressure on your cock and you will find that this allows the cum to come out for a creampie.

Don;t release all pressure completely as your cock may start to slip out but just release around 50% or so.

Do you ejaculate using any other method? Are you on any type of opiods? If you never ejaculate, look up dry orgasm and retrograde ejaculation. There are possible causes listed there.
